Russell, How about checking: 1) Try a situaltion where no castling is allowed e.g. move king to d1 and d8. See if the problem still exists. This will tell you if it is a castling problem. 2) Are you doing repetition detection in the perft routine? I'm not sure what the convention is - is it the norm? Clearly the 4th ply would be when it comes to light. 3) Are you doing castling through check i.e. Black rook on d8 threaten d1 and you castle - probably not the problem since you're not finding enough positions.
