An inside joke, a running gag, and a protest all assume intentionality, and you know what they say about assumptions. Anyway, I don't think there's any reason to be confined by these possibilities. I think in this case the answer is "None of the Above." The notes are probably just out of date. That is probably the most parsimonious and least implicitly hostile explanation.
