The SAN toolkit works fine.  You can't have funny characters (common in European
and Scandinavian names) in the headers, though.  I have a version for NT:

EPD2DIAG works fine for small sets.

I typically run PGN files through a filter to remove funny characters (below 32
except whitespace, above 127...) and then feed it to SAN.  Usually, I will have
to hand-edit a couple of the games in a large collection to get it to work.
Once in a great while, the san toolkit will refuse to process a game, and then I
just give it to EPD2DIAG.

An even better way (but it is not free) is to use BOOKUP.  BOOKUP can read in
PGN files and spit out EPD.  It has a lot of neat features for that.
