I had only a paper copy and I can't find it at the moment.  As I recall there
was the occasional and humorous profanity appearering in the Lisp source
commentary, so perhaps it was not meant for general publication.  Great reading,
though.

Perhaps you should contact SRI and see if you can reach Wilkins there.
