The second, and similarly with the rook, has happened several times in tournament games. I played a tournament game when quite a strong player (Elo 2000) stubbornly refused to play O-O-O - when it would've been the saving move - and lost. When asked afterward it turned out thought they couldn't castle when b8 was attacked by a White piece ... ! Alastair
