[D]r1bq1rk1/pp3pp1/2p1n2p/4PN2/1b6/2NB3P/PP3PP1/R2QR1K1 w - - 1 21 Rizzitano 2485 - Wolfe 2353, Cambridge 1984 The position is from 'Understanding Your Chess' by James Rizzitano. He played 21. Qg4!! without realizing the bishop was hanging. One kindly interpretation might be to argue that his pattern recognition overrode any calculation effort :) I can recommend the book for both humans and as a source of test positions for both find and avoid moves.
