Schlechter missed a win and later a draw to lose the game. In 1985 Karpov with white pieces had to win the last game against Kasparov to hold his title, but he lost the game (the game is remembered by every Sicilian Scheveningen lover). Two years later, in 1987 it was Kasparov with white pieces who had to win the last game against Karpov to hold his title. He managed to win the game, after several blunders by both sides during the game. I don't remember any successful attempt at winning the last game with black pieces.
