The only exceptions to the above would be cases of a game in progress where the
arbiter (or tournament director) had to penalize the players for getting into a
fist fight or for similar misbehavior.  And there are records of GMs kicking,
spitting, and punching, although I don't recall any specific double forfeitures
except for dual no-shows.
