I read about 2 cases when even kasparov forgot.

One case he lost a game after he sacrificed a piece based on a line that was in
his notebook but forgot how to continue.

I think it was against the french GM.

I remember also that I read that in another game he replaced order of moves in
the olympiad(against bad memory) and lost the exchange and had to fight for a
draw.
