It definitely happened.  We were running on new hardware.  And during that game,
we had massive problems. Turns out one cpu forgot how to multiply by a -1, which
screwed things up.  By the time we realized the hardware was bad, and moved to
another machine, we were dead lost.  There was no provision in the rules for
re-starting a game, although we were allowed to change machines when it became
obvious that it was broken (it was rebooted, but we couldn't log in, then it
wouldn't even reboot, etc.)

We always ran on hardware that had just came off the production floor, and was
in a 'burn-in booth'.  Unfortunately, from time to time, we "burned in" the
machine more than we wanted.  That was life in the fast-lane.  Leading edge
often becomes bleeding edge at a critical point...
