Following the PV will obviously be bad if the PV is bad. That's bound to happen
in some cases.

If you don't notice a general speedup from following the PV, though, something
is probably wrong.

It's possible that you're evaluation function doesn't have very much granularity
(say, if you're just counting material) so any reasonable line will produce the
same score as the PV. Then the "PV following heuristic" will obviously offer no
improvement.
