It takes an awful lot of positions to make a decent opening book. Here is how it is usually done: A big pile of PGN is read in. The positions are tagged with wins/losses/draws and saved out to a certain depth in plies. Enhancements: 1. Only save positions that are played a lot and by good players 2. Update book so that a notation of ? or ?? in the incoming data causes "never play", and a notation of !! or ! causes "always play." 3. Update book after playing by tagging losing lines. 4. Update book after playing by tagging winning lines. 5. Update book with analysis data from GM's. 6. Update book with analysis data from computers. There are opening books with literally thousands of hours of effort put into them. These books give the programs that use them an enormous advantage. Even so, I believe that all of them have bugs.
