The entire board position has to be repeated three times *and* every time the player who moves has to be the same. I'm not sure if castling-rights or en-passant-possibilities affect the counting. For example assume a position where white can castle 0. ... Bxc3+ (first occurence of position, castling possible) 1. Ke1f2 (now white can not castle anymore) 1. ... Bc3d4+ 2. Kf2e1 Bd4c3+ (second repetition of position, but this time no castling possible) 3. Ke1f2 Bc3d4+ 4. Kf2e1 Bd4c3 (third repetition and draw or second repetition if castling affects counting) 3.
